(CNN) – President Barack Obama has been briefed on the latest report from the Veterans Affairs inspector general, which found "serious" problems at the VA medical facility in Phoenix, with patients at risk of being "forgotten or lost" in a maze of convoluted scheduling practices.
"The President found the findings of the interim report deeply troubling," says Deputy National Security Adviser Tony Blinken, adding that Obama urged the VA to "act now" to address problems, and not to wait for findings from interim or final reports.
